To add another diary entry for the same clinical term in EMIS Web, follow these steps:

  • Navigate to the Diary section in the Care Record.
  • If no patient is selected, choose one when prompted.
  • Click Add and select Diary Entry.
  • In the search field, type the desired clinical term and press Enter.
  • In the Date section, specify a date by either typing it, using shortcuts like "3m" for three months, or by selecting it from the calendar. If the date is unknown, type "NK".
  • For Patient Age, input the age of the patient if required.
  • In the HCP field, select the clinician from the list or use the Find Users screen to locate them.
  • Enter any specific notes in the Diary Text field.
  • Click OK to save the entry to the patient’s record.
EMIS Web displays only one diary entry for each clinical term, ensuring the most recent information is easily accessible. Older entries remain stored in the patient’s full history.
