Submitting a Yellow Card report in EMIS Web is a critical function for healthcare professionals managing suspected adverse drug reactions (ADRs). This functionality connects directly to the Medicines and Healthcare products Regulatory Agency (MHRA), ensuring timely consideration of potential safety concerns.

Accessing the Yellow Card Functionality

Begin by navigating to the Medication menu from within the patient's care record. If a patient is not yet selected, use the Patient Find feature. On the ribbon, click the Yellow Card icon to open the Yellow Card screen. This interface allows you to report new incidents or review previous submissions.

Filling the Yellow Card

The process to complete a Yellow Card involves:

  • Selecting the patient details, which will auto-fill from the patient's record, and continue by entering or confirming required fields such as weight and height.
  • Entering the relevant drug information, including name, dosage, and suspected reactions. This step is crucial to accurately document the ADRs.
  • Describing associated symptoms and management in the narrative fields provided, ensuring all details that could help in analysing the reaction are included.

Completing and Submitting the Report

Once all necessary fields are filled, review the information entered to ensure accuracy. Click Send to submit the Yellow Card to the MHRA. Should you need to end a medication course due to an ADR, select the medication, right-click to choose End Course, and follow the provided prompts to document the adverse reaction.
